We collect information for the primary purpose of providing quality health care. We require your personal details and medical history so that we may proactively assess, diagnose and treat you.
You are not obliged to provide any information requested of you although failure to do so may compromise the quality of your health care and treatment. You have the right to access information collected about you, except in circumstances where access may legitimately be withheld. In this instance, you will be given an explanation for withholding.
If your information is to be used for any other purpose other than set out above, your further consent will be obtained.
Cancellation fee
When an appointment is made, our practitioners reserve time, space and energy to care just for you. If you are unable to attend your scheduled appointment, we request that you provide us with 24 hours notice so that we may offer your appointment time to another person who is in need. This notice also allows our practitioners to plan their time efficiently. Failure to provide 24 hours notice to change or cancel your appointment will attract a charge equal to 100% of the regular appointment fee.
